Ontario’s Primary Care Action Plan is underway, helping connect residents to family doctors and nurse practitioners across Toronto. Residents can register with Health Care Connect or explore local clinics accepting new patients Ontario Health Teams are supporting primary care access across the city.

Primary Care Access by Area:

Central West OHT: Serving residents in North Etobicoke
Downtown East OHT: Serving residents south of Bloor to Lake Ontario, between the Don River and Yonge Street
East Toronto Health Partners OHT: Serving Flemingdon Park, Thorncliffe Park, East Danforth, Oakridge, Clairlea, Golden Mile, and surrounding neighbourhoods
Mid-West Toronto OHT: Serving Davenport-Perth and surrounding areas
North Toronto OHT: Serving Forest Hill, Lawrence Park, Yonge-Eglinton, Humewood-Cedarvale, and surrounding neighbourhoods
North Western Toronto OHT: Accepting new clients in select postal codes (e.g., M3J, M6M, M9P and others)
North York Toronto Health Partners OHT: Accepting new clients through the Get Well Health Team (priority for postal codes M2N and M3H)
Scarborough OHT: Serving Scarborough residents
West Toronto OHT: Serving High Park, Parkdale, Mimico, Mount Dennis, and surrounding neighbourhoods.
